Homeschool Program

REGULAR HOMESCHOOLERS

 

  1. Birth Certificate

  2. Previous Year's Report Card

  3. Transcript/ SF10 (to be submitted by October)

  4. Clearance from Financial Obligation/ Certificate of Good Financial Standing from previous school/ provider

  5. ID Photo

INDEPENDENT HOMESCHOOLERS

 

  1. PEPT Certificate of Rating

  2. Birth Certificate

  3. Previous Year's Report Card

  4. Transcript/ SF10 (to be submitted by October)

  5. Clearance from Financial Obligation/ Certificate of Good Financial Standing from previous school/ provider

  6. ID Photo
